Chapter Event CEU Submission Form

Please complete this form in full to register your upcoming event for continuing education credit. To receive credit for the event, the Chapter Officer/Organizational POC must:

  1. Provide CMAA with a registration list (including first name, last name, email address, address, city, state, zip, country). The registration list MUST be submitted as an excel document using this excel template
  2. Ensure the required slides are displayed at the function
  3. Require all attendees wishing to receive continuing education credit (PDHs, AIA LUs, CCM Recertification points) sign in. ​You are required to return the completed sign-in sheet to CMAA. ​
  4. Return required documents to CMAA -

The registration list must include complete contact information. The sign-In sheet, with attendee signatures or initials, is used for proof of attendance and will be kept on file by CMAA in case of audit by RCEP, AIA, or CMCI. All three of the above components are required in order for CMAA to submit attendee continuing education credits on your behalf.


DISCLAIMER: Not all courses and educational activities offered will be accepted for continuing education credit in your state.  Please check your state licensing board’s requirements before submitting your credits. All courses approved by CMAA count toward CCM recertification points.
