Chapter Event CEU Submission Form
Please use the form below to submit CMAA Chapter events or CMAA presentations for your organization for continuing education credit.
Please complete this form in full to register your upcoming event for continuing education credit. To receive credit for the event, a Chapter Officer/Organizational POC will need to ensure the RCEP master slides are displayed at the function, and that all attendees wishing to receive RCEP credit fill out the attendance form in full. You are required to scan/mail the sign-in sheet and email the completed Attendance Form (excel only).

The Attendance Form is a registration list with complete contact information as required by RCEP. The Sign-In Sheet, with attendee signatures or initials, is used for proof of attendance and will be kept on file by CMAA in case of audit by RCEP. All three of the above components are required in order for CMAA to submit attendee PDHs on your behalf.
Note: the attendance form must be filled out electronically and returned after attendance has been taken on-site. Forms and sign-in sheets should be sent to firstname.lastname@example.org.
